If you are preparing for your first visit to Ballard Acupuncture Center, please call in to book an appointment. Once you've booked your appointment you will receive a confirmation email with online forms that you will need to fill out. If you are unable to fill out the online forms please allow an additional 20 minutes prior to your scheduled appointment to complete them in the office.
